When we quote for a cake we use the following slice sizes 2 inch / 5cm  square. 

Any special dietary requirement  ( gluten free,  dairy free etc.)

Delivery post code ( there will be a delivery charge )  or indicate if you would like to collect

Type of cake  ie sponge, fruit , chocolate

And an Idea of the design you require.
